CHS Alternate Travel Plans Request

If you need to make alternate travel plans for your student to leave after contest

Then please download this form and get it to the Band Director Staff before Friday of the week of the contest.  Without this form on file with Mr. Mason, your son or daughter will not be allowed to leave with anyone other than the band buses.
